Individuals

This form will generate only a single voucher. If you need multiple vouchers you will have to submit this form for each cat. If you would like assistance with submitting the form for a large number of cats, please email us at fixutah@bestfriends.org.

Best Friends prioritizes support to communities with the largest life saving gaps. The amount of support provided may be provided on a case-by-case basis and is dependent on what community the cats are in.

If you are interested in applying for support, please fill out this form. Once complete, you will receive a link to a voucher. You can take your voucher to any participating veterinary partner.

Our program prioritizes areas that are not covered by a municipal shelter's TNR program. If you are in area that is not currently supported by our program you will receive notification via email after submitting your application.

Shelters

Please use this form to generate vouchers after an appointment is scheduled for the cats. This new system will generate and send them to you within the hour.

Thank you for helping decrease the unwanted cats & kittens in your community. This form will allow you to automatically generate the vouchers needed for SNR.

You can put up to 10 cats on a single voucher. If you need more for a single visit/day please submit a second form.

Can you humanely trap the cats?
Can you transport the cats to the vet?
Our Vet Partners require community cats (feral and friendly) to be transported in a humane trap lined with newspaper and covered with a sheet.
Important: Voucher are only for one cat, this question is asking general information regarding the colony. Please provide an approximate number if not sure.

Voucher Cost and Copay Contributions

The base rate for all vouchers is $20 
(the amount to be paid by you)

This rate may be discounted after submission based on the information provided above. You can also elect to provide a "contribution" to supplement the copay which will be added to the base rate and paid to the clinic at time of service. This contribution helps us offset the cost and extend this service.

If able, pick an amount you're able to donate towards the cost of surgery and vaccinations. This offsets the cost of each surgery to Best Friends and allows us to help more in our community.
